The Coach Sarna 49ers Flag Football League offers 5v5 and 7v7 flag football, along with activities such as the 49ers Flag Skills Camp, NorCal Elite Flag Football Tryout, and a FEMALES IN FLAG CLINIC. The program also includes MLS GO Soccer, LEGO STEM competitions, and a LEGO STEM League. Players participate in non-contact flag football games that follow specific 5v5 and 7v7 formats and rules.

In 5v5 flag football, games are played on a 25-yard-wide by 60-yard-long field with two 15-minute halves, a 3-minute halftime, and two timeouts per game, and all five offensive players, including the center, are eligible to run routes. In 7v7 flag football, games are played on a 60-yard-long by 40-yard-wide field with two 15-minute halves, a 3-minute halftime, and two timeouts per game, and all seven offensive players, including the center, are eligible to run routes. Both 5v5 and 7v7 games start at the 5-yard line, have no tackling, diving, or blocking, use four plays to advance past midfield and score, and use the same scoring system of 6 points for touchdowns, 2 points for safeties, and extra points from the 5-yard line for 1 point or the 10-yard line for 2 points.
At registration, players receive an NFL Flag Jersey that could be from any of the 32 NFL teams, NFL Flag shorts, NFL Flag belts, a 49ers Flag shirt, and a 49ers Flag drawstring bag. Coaches receive a 49ers Flag coach shirt, a 49ers Flag drawstring bag, a ball bag, disc cones, two footballs per team, an in-person coach clinic, annual training at Levi Stadium beginning in late 2026/2027, optional premade practice schedules, optional premade playbooks, and access to drills videos. Coaches receive team uniforms and distribute them before opening weekend at practice.
Division-winning teams receive 49ers championship rings. Families receive preference for 49ers tournament and camp participation and the opportunity for NFL Flag 49ers Tournament entry. The league uses Team Snap for registration, communication, schedules, standings, and drills for coaches.
All players are required to participate in a Double Good Popcorn fundraiser or pay an additional opt-out fee. The league sets a minimum fundraising goal of $55,000 for the season to cover field rentals, officiating, field paint, and equipment, and aims for each player to sell at least $100 in popcorn. The fundraiser uses the DoubleGood Popcorn App with fundraiser code NBFQKX, and players who sell over $100 in popcorn qualify for a Disneyland vacation awarded to the top three selling families. The top selling family can choose either three 2-day Disneyland passes or four tickets to any 49ers home game with unlimited food and beverage and a parking pass, the second place family can choose either two 2-day passes or three tickets to any 49ers home game with unlimited food and beverage and a parking pass (for a game not chosen by the first place family), and the third place family can choose either one 2-day pass or two tickets to any 49ers home game with unlimited food and beverage and a parking pass (for a game not chosen by the first or second place families).
The Coach Sarna 49ers Flag Football League is part of The Coach Sarna League, which is a 501(c)(3) family-oriented recreation league that focuses on teaching football fundamentals and offering a safe and competitive environment to play non-contact flag football. Its mission states that any child who wants to play will be able to play regardless of financial circumstances, and it offers scholarships to every child who needs one and reports that it has not turned away a family due to finances. The league is open to the public, and anyone can register regardless of where they live.
The league highlights community involvement by stressing team bonding events, attendance at community events, and volunteerism among families, including an annual volunteer trip to Door of Faith Orphanage in Mexico. The leadership team includes Ryan Sarna, the CEO and Founder of The Coach Sarna League, and Ed “Coach” Sarna, a longtime local coach, athletic director, and Notre Dame football player for whom the league is named.

At the 2023 NFL Flag Summit in Atlanta, it was nominated for League of the Year and Community-Based League of the Year, and Ryan Sarna was invited as a guest speaker for two segments at the 2023 Annual NFL Flag Conference. Out of over 2,000 NFL Flag Football leagues, The Coach Sarna League was nominated as a 2023 League of the Year Award nominee. At the 2026 RCX Summit in Orlando, the league was nominated for the top league in the nation award for both NFL Flag and MLS GO, won the award for MLS GO, was runner-up for top NFL Flag League of the Year, and Coach Ryan Sarna was a special guest presenter at the conference.
